Issue Description:    The specification does not mention externalFlow in the dtbook-2005-2.dtd.

This Issue Has Been Resolved as an Errata

The revised 2005-3 DTD adds a new entity. This is not in section 4.2.2. We will mention this in the errata section of the site.
