Declarations of Admission (Application Forms)
Instructions for Applying for Membership
In order to successfully submit your application to become a member of the DAISY Consortium, follow these steps:
- Review DAISY's Articles of Association
- Select one of the two file formats of the appropriate declaration form provided below.
- Save your preferred version to your computer.
- Complete the form.
- Be sure to type your name and the date at the bottom of the form.
- Send the declaration to become an associate member, as a file attachment, to the email address president@daisy.org.
- Print the file.
- Sign it.
- Send a copy by postal mail to the mailing address on the form.
We thank you for sending both an email and postal mail version. A signature is required for our official files.
Shortly after your emailed form has been received, you will be contacted by the DAISY Consortium. The Consortium's Treasurer will invoice you for the required fee. Once your payment has been received, you will be provided with a login to the DAISY Web site.
Declaration on Admission
Please choose the appropriate Declaration on Admission form for your level of membership. We provide both a version in Microsoft Word or plain text for your convenience:
