Section 2.4.1.2 says that "If a Publication includes an NCX, the item that describes the NCX must be referenced by the spine toc attribute." This seems to imply that the NCX is optional for Publications. Perhaps the phrase should be removed so that the new wording would be: "The item that describes the NCX must be referenced by the spine toc attribute.
